Here's What You Should Know
Folate (vitamin B9) is required for DNA synthesis, cell division, and one of the most critical pathways in human metabolism — the methylation cycle. Methylation is how the body processes neurotransmitters, detoxifies estrogens, regulates gene expression, and rebuilds myelin. When folate is insufficient, all of those processes slow — and the downstream consequences include elevated homocysteine (a cardiovascular and neurological risk marker), impaired mood regulation, and compromised cellular repair.
The distinction between folate and folic acid is clinically important. Folate is the natural, food-derived form. Folic acid is the synthetic form used in supplements and food fortification. Approximately 40–60% of people carry at least one variant of the MTHFR gene that reduces their ability to convert folic acid into active 5-methyltetrahydrofolate (5-MTHF). For these individuals, standard folic acid supplementation has limited effectiveness — and only the active methylated form (5-MTHF, also called methylfolate) reaches cellular destinations.

Why This Gets Missed
MTHFR variants are extremely common and almost never screened in routine care. Someone with a double C677T variant may have folate "in normal range" on a standard serum test but remain functionally deficient because they cannot convert it to the active form. Methotrexate (used for rheumatoid arthritis, psoriasis, and some cancers), oral contraceptives, anticonvulsants, and metformin all deplete folate. Alcohol significantly reduces folate absorption and increases urinary excretion.
How It Connects to the Bigger Pattern
Folate works as a unit with B12 and B6 in the methylation cycle. Deficiency in one stalls the others. Elevated homocysteine — the signal of a stalled methylation cycle — is associated with cardiovascular risk, neurological inflammation, and accelerated cognitive decline. The pattern often isn't a single deficiency; it's a compounded shortfall across all three B vitamins feeding the same pathway. People on long-term methotrexate, oral contraceptives, or anticonvulsants need active monitoring.
What to Start With
Dark leafy greens (spinach, romaine, arugula), legumes (lentils, black beans), avocado, and asparagus are the highest natural folate sources. Cooking destroys a significant portion — raw or lightly steamed is better. For supplementation, choosing methylfolate (5-MTHF) rather than folic acid is meaningful for anyone with MTHFR variants or poor response to standard B vitamins. Testing both serum folate and plasma homocysteine gives a more complete functional picture than serum folate alone.
