Here's What You Should Know
Vitamin B6 (pyridoxine, and its active form pyridoxal-5-phosphate, P5P) is a cofactor in over 100 enzymatic reactions in the body — more than almost any other single nutrient. Among its most critical roles: it converts tryptophan into serotonin, tyrosine into dopamine, glutamate into GABA (your primary calming neurotransmitter), and it's required for the synthesis of histamine, which regulates immune response and stomach acid production. It is also essential for the conversion of homocysteine — the cardiovascular risk marker — into harmless byproducts.
When B6 is low, neurotransmitter production becomes erratic. Mood becomes unstable. Sleep deteriorates. Anxiety increases. PMS worsens. These symptoms are common enough to seem non-specific — but they're frequently downstream of a depletable, replaceable nutrient.
Signs This May Be Part of Your Pattern
- Mood swings, irritability, or low mood — especially in the premenstrual phase
- Poor dream recall or vivid, disturbing dreams (B6 is required for dream-stage sleep processing)

Why This Gets Missed
Oral contraceptives are one of the most significant B6 depleters — the pill increases the enzymatic pathway that consumes B6, reducing plasma levels measurably and consistently. Many women experiencing mood disruption, anxiety, or worsened PMS on the pill are experiencing downstream B6 depletion without knowing it. Other depleters: isoniazid (TB treatment), hydralazine, corticosteroids, and loop diuretics.
The distinction between pyridoxine (standard supplement form) and pyridoxal-5-phosphate (P5P, active form) matters for people with liver dysfunction or those who cannot efficiently convert — P5P bypasses the conversion step.
How It Connects to the Bigger Pattern
B6 is a core component of the methylation pathway alongside B12 and folate. Deficiency in one typically means the others are under pressure. B6 deficiency elevates homocysteine independently of B12 and folate status. In women, B6 acts directly on progesterone signaling and estrogen metabolism in the liver — which is why clinical research shows significant PMS symptom reduction with B6 supplementation, especially breast tenderness, mood, and fluid retention. Women on contraceptives with PMS symptoms are often B6-deficient and respond well to P5P supplementation.
What to Start With
Chicken, turkey, tuna, salmon, potato with skin, and bananas are among the best dietary sources. Cooking significantly degrades B6 — particularly boiling. For supplementation, 25–50mg of P5P is adequate for most people. High-dose pyridoxine (over 200mg/day for extended periods) can cause sensory neuropathy — more is not better and long-term high doses require monitoring.
